Douglas County Health Center has proudly served the Omaha area for over 100 years and continues to be a leader in health care in our community. Our team of healthcare professionals provides excellent Short Term Post-Acute Rehabilitation and Long Term Care. We are recognized by the Centers for Medicare and Medicaid as a Five Star Rated facility. We are proud to be the only facility in Nebraska to hold that rating consistently since the inception of the rating program in 2008. Douglas County Health Center is nestled in the Historical Field Club neighborhood and is centered between two of Nebraska’s best-known hospitals and National leaders in acute care; The Nebraska Medical Center and the Omaha Veterans Medical Center. We are conveniently located in Midtown close to major shopping areas, the Old Market, interstate, and the airport.

Memory Support

Our staff understands that caring for someone with Alzheimer’s or Dementia can be challenging. When care can no longer be provided at home, our staff at Douglas County Health Center invites you to entrust your loved one’s care to us. Our Memory Support Program creates a comfortable and secure environment that both the individual and their family will enjoy.

Rehabilitation and Transitional Care

Our experienced therapy team is committed to providing outstanding results and excellent quality care. Patients are empowered through individualized programs to reach goals that will increase their independence and ability. Our team also encourages families and caregivers to participate in therapy sessions, caregiver education, patient, and training.

Hospice Care

We offer hospice care for those whose illness, disease or condition is unlikely to be cured. We will evaluate so that we can provide the most comfortable and appropriate care plan.

Activities Program

A positive and uplifting atmosphere is vital to the success of healing, recovery, and rehabilitation. Douglas County Health Center employs outgoing and friendly professional staff to create social programs and activities to fit the residents’ needs. We provide a monthly activity calendar to each resident to keep them informed of upcoming events. Friends and family are always welcome to visit and attend our special events.
